The 4-Day Rule Explained

Tirzepatide is dosed once weekly, every 7 days. The 4-day rule from FDA prescribing information works like this:

If you realize you missed a dose:

  • Count from your scheduled injection day to today
  • If 4 or fewer days have passed since your scheduled day → inject now
  • If more than 4 days have passed since your scheduled day → skip this dose

Then in both cases: resume your regular weekly schedule on the next scheduled injection day.

Example 1: Short miss

You inject every Monday. It's now Thursday, 3 days since Monday. Inject today (Thursday). Then inject again next Monday as usual. No change to your schedule.

Example 2: Long miss

You inject every Monday. It's now Saturday, 5 days since Monday. Skip the missed dose. Inject next Monday as usual. You simply went 12 days between doses this cycle instead of 7.

Why You Cannot Double Up

Never inject two doses at once to make up for a missed one. Doubling your dose doesn't "catch up." It dramatically increases side effects, especially nausea, vomiting, and GI distress. It can also cause blood sugar fluctuations in patients with type 2 diabetes.

The pharmacokinetics of tirzepatide don't work like a simple cumulative drug. A single dose of 10 mg is what your system is calibrated for. 20 mg at once would be a significant overshoot.

How a Missed Dose Affects Your Schedule

The 4-day rule is designed to keep you on your original day of the week regardless of when you inject the missed dose.

If you inject on the recovery day (say, Thursday instead of Monday), you then wait to inject again on the following Monday. That's a 4-day gap that one time instead of 7 days. This is acceptable. The following week and all subsequent weeks return to the standard Monday schedule.

Your weekly injection day does not change just because you missed one dose.

What About Dose Escalation Timing?

The tirzepatide dosage schedule calls for 4 consecutive weeks at each dose before escalating. If you miss a week during an escalation phase, does that reset the 4-week clock?

The short answer: discuss with your prescriber, but one missed dose in a 4-week phase doesn't typically require restarting that phase. Most providers consider 3 out of 4 doses in a phase sufficient to proceed with escalation as scheduled.

How Missed Doses Affect Weight Loss

One missed dose won't derail your results. The half-life of tirzepatide means active drug is still present in your system for 10–14 days after a single dose.

Consistent weekly dosing, however, does matter for long-term outcomes. The SURMOUNT-1 trial results, which averaged 22.5% weight loss at 15 mg, assumed weekly dosing. Irregular dosing means inconsistent plasma levels and inconsistent appetite suppression.

The practical effect of 1–2 missed doses per month is an estimated 10–20% reduction in medication effectiveness over time, based on pharmacokinetic modeling. Not catastrophic, but noticeable in your weight trajectory.
